Covering letter addressed to the Assistant Director of Drugs Control of Concerned zone along with Rs.2/- court fee stamp for each licence.

Form 19C – to be filled and downloaded at e-seva centre and the same shall be duly signed and uploaded at e-seva centre.

Authorization document, in case of application signed by the authorized signatory

Declaration form duly filled & signed by applicant & pharmacist.

Copy of drug licence held by the applicant – if any

a.Ownership document of the premises

b.Plan of the premises duly signed by licensed surveyors and the applicant.

c.Rental agreement of the premises, if applicable

Pharmacist/ competent person’s details:

a.Pharmacy council registration certificate

b.Education qualification certificate

c.Experience certificate of competent person

d.Declaration of the pharmacist

a.Document relating to constitution of concerned firm/ Company/ LLP and others.

b.In case of proprietor/ prorietrix a passport size photo (file size not more than 50 kb), in case Partnership/ LLP- group photo (file size not more than 50 kb)

c.Address/ ID proof of the applicant.

d.Legal tenancy affidavit

Purchase bills of A/C and refrigerator along with working condition/Installation Certificate.

Cupboard/drawer with lock and key

Enclose original licence

Request to cancel the licence issued to old premises

Documents related to change in constitution like sale deed, Dissolution/ Reconstitution deed, Amalgamation court order if it is a Company along with board resolution and any other document.

If the existing licence is obtained through online then grant due to change of premises/ change in constitution should be applied through e-Sevai centre only.

In case of licence issued prior to launch of online process then application has to be submitted manually in the concerned zone.

Storage facilities racks, Refrigerator, A/C and Cupboard/drawer with lock & key has to be provided.

The applicant shall visit e-Sevai Centre (Click here for list of all e-Sevai Centres in Tamil Nadu) and provide details requested in the application form to the computer operator. The computer operator will fill the application on the online system on behalf of the applicant. The applicant shall download the duly filled Form 19 at e-Sevai Centre and he shall sign and upload the same at e-Sevai Centre.

Once documents are uploaded, an application number will be generated and the applicant will receive a confirmation of the same via SMS. The requisite fee is to be paid by the applicant/ firm

The applicant shall send the hard copy of documents via speed post/ courier, quoting the application reference number, to the concerned Approving Authority (Assistant Director of Drugs Control (ADDC) of concerned zonal office). The ADDC will scrutinize the application within 3 working days:

a.If the application is in complete shape, it will be forwarded to the Drugs Inspector for inspection report.

b.If the file is incomplete, the required particulars/documents will be requested from applicant through online system by the approving authority within 7 working days. And after the receipt of required particulars, the application will be forwarded to the Drugs Inspector for inspection report.

The Drugs Inspector will inspect the premises and will upload his recommendation/ remarks on the online portal. The uploaded recommendation/ remarks will then be forwarded to the concern ADDC for recommendation within 7 working days.

After the receipt of the application with recommendation/ remarks from the Drugs Inspector, the Licensing authority(ADDC) will scrutinize the application:

e)If the application is in complete shape, the License will be issued within 7 working days.

f)If the recommendation of Drugs Inspector is for rejection, the Approving authority will take necessary followup action for rejection within 7 working days and the same will be communicated along with reason for rejection to the applicant through online system

The applicant can download the license from e-Seva Centre after the receipt of the SMS alert.
